Valinoti , Assistant Director <br /> RE: Underground Storage Tank Program <br /> Abandon Tank Policy (Tanks Abandoned prior to 7-1-84) <br /> The above referenced abandoned tank policy will be implemented by Environmental <br /> Health Staff according to one of the following alternatives: <br /> Alternative #1 <br /> 1. Provide a copy of the Fire Department (District) Permit to the San Joaquin <br /> Local ,Health District Division of Environmental Health. <br /> 2. Provide a plot plan showing the name of property owner, facility address, <br /> parcel number, tank size, product stored, method of abandonment and tanks) <br /> distance to all wells within 1000 feet of the abandoned tank(s). <br /> Alternative #2 <br /> 1. Provide a recor-d-of materials purchased to abandon tank(s) . <br /> 2. Provide a plot plan showing the name of property owner, facility address, <br /> parcel number, tank size, product stored, method of abandonment and tank(s) <br /> distance to all wells within 1000 feet of the abandoned tank(s). <br /> Alternatfve,#3 <br /> 1. Provide a signed statement from the property owner stating date tank abandoned. <br /> 2. Provide a .plot pian showing .the name of property owner, facility address, <br /> parcel number, tank size, product .stored,- method of abandonment and tank(s) <br /> distance to all .wells within 1000 feet of the abandoned tank(s). <br /> All underground storage tanks abandoned after 7-1-84 shall follow the procedures <br /> set by State Law.
