SUMMARY <br /> On April 30, 2003, Mr. Dennis Catanyag, Registered Environmental Health Specialist <br /> (REHS), San Joaquin County Environmental Health Department (EHD), conducted a <br /> routine hazardous waste inspection with Ms. Michelle Le, REHS, EHD, at the above <br /> referenced facility. Three hazardous waste containers were not labeled properly and <br /> several containers of hazardous waste were not closed and not in use at the time of <br /> inspection. A routine inspection report was issued (Attachment 1) to Mr. Oren C. Tuss, <br /> Operations Manager. On June 30, 2003, a certification of return to compliance was <br /> received without any other supporting documentation from Mr. Tuss (Attachment 2). <br /> On June 4, 2004, Ms. Le conducted a routine hazardous waste inspection at this facility. <br /> Three hazardous waste containers were not properly labeled and three 5-gallon containers <br /> of hazardous waste were not closed and not in use at the time of inspection. These are <br /> repeat violations from the previous inspection. A 5-gallon container holding used batteries <br /> was not labeled with an accumulation start date. The code section for this violation was <br /> incorrectly noted on the inspection report as violation#75, but should have been marked as <br /> violation #76. An inspection report was issued to Mr. Tuss (Attachment 3). On July 2, <br /> 2004, Mr. Tuss submitted a corrective actions statement without a certification of return to <br /> compliance(Attachment 4). <br /> On June 28, 2005, Ms. Le conducted a routine hazardous waste inspection at this facility. <br /> Several hazardous waste containers did not have proper labeling and two 5-gallon <br /> containers of hazardous waste were not closed and not in use at the time of inspection. <br />
