<br /> commitments and surveys for the use of a portable HDR to be provided by a transportable service company,
<br /> RML 6152.
<br /> (1) The letter dated June 22, 1998, as supplemented by the letters dated August 20, 1998, January 26, 1999, and the
<br /> letter dated October 6, 1998, with attachments, all signed by Maria Chan, Ph.D., DABMP, Radiation Physicist,
<br /> Radiation Safety Officer, regarding the addition of use location to do radiolocalization procedures with Tc-99m.
<br /> (m) The letter dated July 21, 1999, and the letter, with attachments, dated October 8, 1999, and the letter,
<br /> with attachments, dated October 26, 1999, all signed by Jiun-Der Chern, M.S., Radiation Safety Officer,
<br /> as supplemented by the letter, with attachments, dated July 28, 1999, signed by Jiun-Der Chern, M.S.,
<br /> Radiation Safety Officer, and Donald J. Wiley, Chief Operating Officer, regarding vascular
<br /> brachytherapy commitments and procedures and duties and responsibilities of the Radiation Safety
<br /> Officer.
<br /> 14. (a) The Radiation Safety Officer in this program shall be Jiun-Der Chern,M.S.
<br /> (b) The Chairperson of the Radiation Safety Committee shall be Royce J. Biddle, M.D.
<br /> (c) The Custodian of sealed sources shall be Jack L. Funamura, M.D.
<br /> (d) The Alternate Radiation Safety Officers in this program shall be M. L. Del Paine, M.D. and Royce J. Biddle,
<br /> M.D.
<br /> 15. Sealed sources possessed under this license shall be tested for leakage and/or contamination as required by Title 17,
<br /> California Code of Regulations, Section 30275 (c).
<br /> 16. The following individuals are authorized to collect wipe test samples of sealed sources possessed under this license
<br /> using leak test kits acceptable to the California Department of Health Services:
<br /> (a) The Radiation Safety Officer
<br /> (b) Qualified individuals designated in writing by the Radiation Safety Officer
<br /> 17. Except for alpha sources, the periodic leak test required by Condition 15 does not apply to sealed sources that are
<br /> stored and not being used. The sources excepted from this test shall be tested for leakage prior to any use or transfer
<br /> to another person unless they have been leak tested within six months prior to the date of use or transfer.
<br /> 18. The licensee shall conduct a physical inventory every six months to account for all sealed sources and/or devices
<br /> received and possessed under the license. Records of the inventories shall be maintained for inspection, and may be
<br /> disposed of following Department inspection.
<br /> 19. The licensee is authorized to hold radioactive materials with a physical half-life of less than 65 days for decay in
<br /> storage before disposal in ordinary trash provided:
<br /> (a) Radioactive waste to be disposed of in this manner shall be held for decay in storage for at least 10 half-lives.
<br /> (b) Before disposal as normal waste, radioactive waste shall be surveyed to determine that its radioactivity cannot
<br /> be distinguished from background. All radiation labels shall be removed or obliterated.
