PURPOSE: <br /> An inspection was made of R V Circuits due to a request by <br /> Dennis Fields, OES, following his observation of numerous <br /> violations existing at the facility. Dennis Fields stated that <br /> in an earlier inspection Mr. Smith had implied that other <br /> agencies had approved the conditions which were in obvious <br /> violation of fire and hazardous waste codes. <br /> II. INVESTIGATION: <br /> The investigation began at 0911 hrs, in the office of Art <br /> Smith, President of R V Circuits. Mr. Smith introduced <br /> himself and provided business cards from his desk with his <br /> name and title.
