CORRECTIVE ACTIONS: 1. A five-sided Plexiglas box will be constructed and placed <br /> over the high pressure shut-off switch. This Plexiglas box <br /> is intended to direct the flow of any TDI being released <br /> from the switch directly down into the secondary <br /> containment pit. If a similar diaphragm were to rupture, the <br /> clean-up time will be reduced and the possibility of TDI <br /> spreading beyond the secondary containment pit eliminated. <br /> 2. The TDI circulation loop will be shut-off during the pour <br /> station lunch period (usually 11:30 AM - 12:00 PM). This <br /> corrective action will ensure that pour station personnel are <br /> present while the loop is circulating. <br /> 3. John Eschmann, the pour station manager, is seeking the <br /> input of Corporate Engineering as to any additional <br /> corrective actions we should consider. <br /> All corporate suggestions will be thoroughly reviewed by <br /> Steve Fabela, Steve Setzer, Ed Ware, and John Eschmann, <br /> prior to taking any further corrective actions. <br />
