Manifest number 006913177JJK (shipped 07/07/10) neither the final signed copy from the designated facility and its generator's initial copy were not available for review. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository, Generator Information Services Section P.O. Box 806, Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. Submit to SJC EHD within 30 days (07/27/11): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for the above violation 3. a copy of the missing manifest Notes: -Five hazardous waste storage areas: Satellite Accumulation #1: waste paint in truck shop. SA#2: waste oil in maintenance. SA#3: pouring department. SA#4: pouring station. Main HW at northeast of plant - In 2010 year, facility generated approximately 17.69 Tons of uniformed manifest hazardous waste. -Vehicle maintenance performed by Idealease of Stockton off-site. Trucks refuel at cardlocks offsite. - Resin beads/urea waste generated from water treatment that will no longer be used. No future generation of this waste. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
