The following was noted at the time of inspection: 1) Various debris(foam, solids, etc.)were observed inside the two concrete lined former chemical storage pits located inside facility building. Mr. Parissi indicated that these pits were pressure washed and the rinsate was disposed of as non-RCRA hazardous waste. Furthermore,the facility intends to fill both of these pits with concrete. A generator shall make a hazardous waste determination and keep a record of any test results, waste analyses, or other determinations made in accordance with hazardous waste regulations for at least three years from the date that the waste was last disposed of. Immediately make a hazardous waste determination for the residual debris located inside these storage pits or provide a previously made hazardous waste determination, and manage all residual wastes according the Title 22 hazardous waste regulations. 2) According to Mr. Parissi, one concrete lined pit(approximately 10 feet in depth),which previously housed process equipment was filled with concrete several months ago. Mr. Parissi indicated that this pit was cleaned out before it was filled with concrete. Please identify to the EHD what chemicals were previously stored inside this containment, and provide proof that this pit was adequately cleaned prior to its backfill. 3) One piece of metal working equipment (located inside the facility)was observed still containing metal fines and metal workings. Immediately make a hazardous waste determination for the metal fines, and manage all residual wastes according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how these wastes were managed. This is a repeat violation, Class IL 115 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. Copies of uniform manifests for the disposal of hazardous waste for three years were not available for review. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility on site for three years and have them readily available for review. Immediately locate a copy of all manifests for the last three years, submit copies of all manifests associated with this facility dismantling operation. This is a repeat violation, Class ll. 301 CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. The following was noted at the time of inspection: 1)A machinery (located inside the former mechanic shop )with an on board lubricant tank was observed leaking oily liquid. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. 2) Concrete staining and debris were observed adjacent to and inside the former permanent hazardous waste storage area. Immediately fix all leaks, staining and debris accumulation , and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. This is a repeat violation, Class II.
