#21) Failed to keep hazardous waste containers closed except when adding or removing <br /> waste. <br /> -Outside, the DE tank was open. This is a violation of CCR, Title 22, Section 66265.173(a). A <br /> container holding hazardous waste shall always be closed during transfer and storage, except <br /> when it is necessary to add or remove waste. <br /> Immediately, provide a way to close this tank. <br /> #27) Failed to keep signed copy of manifests for 3 years. This is a repeat violation. Gary <br /> Cummings was unable to provide any manifests for the past three years. Records of all <br /> hazardous waste disposal shall be kept for 3 years. This is in violation of CCR 66262.40. <br /> #30) Failed to keep a copy of a consolidated manifest for 3 years. This is a repeat <br /> violation. Gary Cummings was unable to provide any consolidated manifests for the past three <br /> years. Records of all hazardous waste disposal shall be kept for 3 years. This is in violation of <br /> HSC 25160.2. <br /> #31) Failed to keep a copy of manifest or bill of lading for lead acid batteries for 3 years. <br /> No records of lead acid battery disposal were available for reviews. Records of all hazardous <br /> waste disposal shall be kept for 3 years. This is in violation of CCR 66266.81(a)(4)(B). <br /> #49) Personnel training records incomplete. <br /> Sierra Chemical was unable to provide personnel training records for the current year. This is in <br /> violation of CCR, Title 22, Section 66265.16(b)(c)(d)&(e). Facility personnel shall be trained in <br /> hazardous waste management procedures within 6 months of assuming their position, they <br /> shall take part in annual training, the owner and operator shall maintain the training documents <br /> at the facility, and the training records for all employees handling hazardous waste shall be kept <br /> until closure of the facility. <br /> ALL VIOLATIONS MENTIONED ABOVE MUST BE CORRECTED WITHIN 30 <br /> DAYS, UNLESS OTHERWISE STATED IN REPORT, AND A RETURN TO <br /> COMPLIANCE CERTIFICATION (provided on the back of page 1) SHALL BE <br /> SUBMITTED TO THIS OFFICE WITHIN 35 DAYS OF RECEIPT OF THIS <br /> REPORT. <br /> This facility is subject to reinspection at any time at EHD's hourly rate. <br /> Received By: Title: <br /> Date Received: <br />
