The training records on site were incomplete. The person who presented the training, Jason Kirtley, <br /> does not have current training. The job title descriptions were incomplete, and included only the <br /> description of typical job duties. Training records for shop employees was not available for training <br /> conducting prior to 2011. Facility personnel shall complete a training program that teaches them to <br /> perform their duties in a way that ensures the facility's compliance with Title 22 regulations. This program <br /> shall be directed by a person trained in hazardous waste management procedures and shall include the <br /> following: <br /> - hazardous waste management procedures relevant to the positions in which they are employed <br /> - contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually.
