Failed to submit a written response within 30 days of receiving an inspection report. A Hazardous Waste official inspection report was conducted 12/8/2011 and a copy was sent via email to the Mgr Environmental Field Ops on December 29, 2011. Neither a written response nor a "Return to Compliance" was received by EHD within the due date by 01/30/2012. An email was sent on 03/12/2012 to state no docurnentation has yet to been received. The certified mail copy of the inspection report along with a Return to Compliance follow-up letter was sent 11/1/12 and was accepted and signed by a representative on 11/06/12. A written response for violations in 2012 year was finally received by EHD on 08/02/13 - approximately 18 months after the due date. 703. Failed to provide a primary and secondary emergency coordinator. Facility lists a dispatch 1-888- phone number for the Nebraska based Response Management Communication Center as an 24 hour emergency contact phone number. No emergency coordinator has been identified for this facility. Per CFR262.34(d)(5)(i), at all times there shall be at least one employee either on the facility premises or on call (available to respond to an emergency by reaching the facility within a short period of time) with the responsibility for coordinating all emergency response measures. This emergency coordinator shall be thoroughly familiar with: - all aspects of the facility's contingency plan (and have the authority to commit the resources needed to carry out the contingency plan) - all operations and activities at the facility - the location and characteristics of waste handled - the location of all records within the facility - the facility layout Immediately appoint an emergency coordinator for this facility. Submit to SJC EHD within 30 days (11/15/13): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for each of the above violations Notes: - CERS ID 10183327.
