NOTIFICATION PROCEDURES <br />Depending on the type of release, handlers of hazardous materials and waste are required <br />to pro vide an immediate verbal report of any significant release or threatened release of a <br />hazardous material or waste to: <br />a. "911"- if emergency response is needed <br />b. San Joaquin County, Office of Emergency Services — (209) 468-6200 <br />C. Office of Emergency Services - (800) 852-7550 — this satisfies the state <br />reporting requirement; obtain the OES control number when calling. <br />d. San Joaquin County Public Health Services, Environmental Health <br />Department — (209) 468-3420 <br />e. National Response Center (800) 414-8892 or EPA "Spill Phone" — (415) 744- <br />2000 — this satisfies federal reporting requirements <br />City of Lodi Wastewater Treatment Plant — (209) 333-6749; Emergency, 24 <br />hours: (209) 368-5735 — if the spill may affect sanitary or industrial wastewater <br />systems, drains etc. <br />g. Regional Water Quality Control Board, Central Valley Region, Sacramento <br />Offices — (916) 464-3291 — if the spill may affect storm drains or bodies of water <br />(rivers, creeks, ponds, etc.) <br />h. San Joaquin Valley Unified Air Pollution Control District — (209) 557-6400 — <br />if the Air Quality Management District regulates the hazardous material (e.g., <br />chemicals that evaporate or form clouds) <br />The Office of Emergency Services (OES) will notify agencies such as the State <br />Emergency Planning Commission (SERC) (also know as the Chemical Emergency <br />Planning & Response Commission [CEPRC]), the Fish & Game Department, and the <br />Department of Toxic Substances Control (state level) as appropriate. However, <br />companies with knowledge that the spill may have adverse effect on health or the <br />environment should take the imitative to notify these other agencies because notification <br />by the OES may be delayed. (The Department of Toxic Substances Control (state level) <br />strongly recommends this.) <br />Example: Notification of the Fish & Game Department would be appropriate if a spill <br />may contaminate a stream or lake. <br />Thule Towing Systems, LLC. 1313 S.
