4. The written report should include the following: <br />a. The control number received when verbal report was made <br />b. Name, address and telephone number of owner or operator <br />C. Name, address and telephone number of facility <br />d. Date, time and type of incident (i.e.: fire, earthquake, etc.) <br />e. Name and quantity of material involved <br />f. Extent of injuries, if any <br />g. Assessment of actual or potential hazards to human health or <br />environment, where applicable <br />h. Estimated quantity of treated, stored, or disposed of recovered material <br />that resulted from the incident <br />5. Report for a fire: <br />a. Exact location of Fre <br />b.
