<br />SUMMARY OF VIOLATIONS
<br />CLASS I, CLASS II, or MINOR -Notice to Comply)
<br />This is an annual routine inspection of the UST facility at the referenced address on June 26, 2014. The
<br />Official Inspection Report of this inspection was mailed to the owner on June 27, 2014. The facility
<br />remains closed and the USTs abandoned. No access to the building is available. The following are
<br />Class[ violations: 101,102,104,105,106,108,109,110,111,113,118,120,201,202,204,205,206,301,302,
<br />303,304,305,311,314,315,317,999
<br />101 & 102. A permit to operate the UST system has not been issued and fees for annual operating
<br />operating permit for the current year and previous years have not been paid. Fees for the annual permit
<br />to operate the UST system have not been paid. No person may own or operate an UST unless a permit
<br />for its operation has been issued by the local agency to the owner or operator of the UST system.
<br />Immediately contact the EHD accounting office to make arrangements for payment to obtain a permit to
<br />operate a UST system.
<br />104. Financial responsibility documents have not been submitted to the EHD. Current financial
<br />responsibility documents are required to be submitted annually. Immediately complete and submit a
<br />copy of the financial responsibility.
<br />105. UST facility and tank forms on file with the EHD are not current. Any change of information on
<br />these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and
<br />submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank.
<br />106. The monitoring and response plans on site are not current and not approved by the EHD. The
<br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy
<br />of the monitoring and response plans for approval by the EHD. An approved copy will be returned for
<br />your records.
<br />108. Secondary containment testing was last performed on 6/26/2008 and was due by 613012011.
<br />Testing has not been completed and is months past due. Secondary containment testing is required
<br />once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD.
<br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
<br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
