Immediately ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />205. Unable to determine if the designated operator monthly inspection reports since the last EHD <br />routine inspection on August 29, 2008 were retained on site. Designated operator monthly inspection <br />reports for the previous twelve months shall be retained on site. Immediately locate and ensure that <br />copies of the all missing designated operator monthly inspection reports since August 29, 2008 are <br />maintained on site. Submit copies to the EHD. <br />206. Unable to determine if the designated operator employee training was performed. The designated <br />operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Immediately ensure that employees have been trained by the designated operator, maintain the list on <br />site. <br />301. UST system is currently not being monitored to prevent unauthorized release. The underground <br />storage tank system shall be operated to prevent unauthorized releases. Immediately begin operating <br />UST system that will prevent unauthorized releases. <br />302 & 304. Unable to determine if annular sensors detecting a leak from primary to secondary <br />containment and activates an audible and visual alarm and, therefore, the tanks are not monitored. The <br />interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm <br />system.
