Van Buren CONTRACTOR #: 39000
Stockton, CA 95203
The attached "Notice of Reimbursement" is official notification that the site has been split from the 432 N.
Madison Street site. This will enable our department to better evaluate the site's mitigative measures.
The Madison Street site currently has 3 underground storage tanks in noncompliance with the
Underground Tank Law (Health & Safety Code, Division 20, Chapter 6.7, Section 25299(b)(3)); whereas,
the Van Buren Street site no longer has underground tanks.
Fees for review activities and inspections associated with contaminated site assessment activities will not be
billed by this office. The State Water Resource Control Board (SWRCB) will invoice you directly for
activities performed under the auspices of the Local UST Oversight Program. The goal of the site
evaluation performed by Public Health Services Environmental Health Division (PHS/EHD) is to certify
closures of underground tank leakage sites. Closure certification occurs only after mitigation activities are
completed.
If you have any questions or there are any corrections in the responsible party information, contact Diane
Hinson, Supervising REHS, at (209) 468-3452.
Jogi Khanna, M.D., M.P.H.
Health Offi
urie A.
