However, prior <br /> to providing site certification there are a few points of clarification that must <br /> be made to the January report. The following items must be responded to prior <br /> to September 16, 1991 in an addendum to the "Subsurface Environmental <br /> Investigation". <br /> 1. Table 1, page 4, lacks the dates respective to the sampling <br /> activities. Please provide a corrected table. <br /> 2. Enclosed is the original permit application for soil borings listing <br /> J-Con as the contractor. We are returning an $89.00 check #286 to <br /> Mr. Andrew Lush as the application was not accepted by our <br /> department. <br /> Please have the contractor, Woodward Drilling Co. , completed the <br /> enclosed permit application and remit a current check in the amount <br /> of $89.00. <br /> 3. The boring logs lack the field gelogist's name and do not bear the <br /> signature of the certifying professional. Please submit corrected <br /> boring logs. <br /> 4. The description of the excavation of the former dispenser area should <br /> detail the amount of soil removed and where the verification samples <br /> were collected (i.e. , bottom and/or sidewall) . <br /> 5.
