V1. CONTINGENCY PLAN <br /> If an injury occurs, take the following action: <br /> o Get medical attention for the injured person immediately, <br /> o Notify the Site Safety Officer and Site Team Leader. <br /> o Depending on the type and severity of the injury, notify the <br /> Corporate Consulting Physician or the occupational physician for <br /> the injured person. <br /> o Notify the injured person's personnel office. <br /> o Prepare the incident report. The Site Safety Officer is <br /> responsible for its preparation and submittal to the Health and <br /> Safety Direction and corporate personnel office within 24 hours. t <br /> o The Site Safety Officer will assume charge during a medical <br /> emergency. <br /> A. Local <br /> Emergency Phone Numbers are to be posted in Safety Officers vehicle <br /> parked on site. <br /> o Ambulance and Hospital. <br /> o Poison Control Center and Sheriff. <br /> o Fire Stations and Hazardous Waste Fire Sections. <br /> B.
