The permit has been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br />District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />3. At least one person who holds the International Code Council "UST <br />Installer/Retrofitter" certification must be onsite AT ALL TIMES when work is <br />performed. <br />4. A "Certificate of Compliance for Underground Storage Tank Installation Form C" <br />shall be submitted by the owner or the owners' agent [pursuant to California Code of <br />Regulation (CCR) 2635(d)] before this upgrade will be considered complete by <br />SJCEHD. <br />5. Contact John Jackson at (209) 468-3440 to arrange the following inspection <br />appointments; a 48-hour notice for inspection appointments is required. <br />a. Corrosion protection: prior to backfilling around the spill buckets, corrosion <br />protection of the risers must be verified. <br />b. Overfill prevention: overfill prevention system in compliance with CCR 2635 <br />(b)(2) must be verified. <br />c. Spill Bucket: after concrete is poured, verify minimum 5 -gallon capacity and <br />conduct a 30 -minute lake test in compliance with CCR 2635(b)(1). Proper <br />drainage must also be verified. <br />d. Third party tank integrity tests must be performed once concrete is poured, <br />prior to the contractor signing off of the jobsite. The test results must be <br />submitted to SJCEHD within 7 days of completing this project. <br />6. Permit expires 90 days from the approval date.
