An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment test on the September 2014, October 2014, November 2014 and December 2014 designated operator monthly inspection report. The secondary containment test dates on the reports were September 23, 2014, the actual test date was September 29, 2014. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain in the 9-10 at Ernies General Store and 9-10 Pacific Pride UDC sumps failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This is a Class II violation. 314 CCR 2631(a) Primary containment not tight (pre -Jul 2003). There was diesel fuel in the 7-8 and 9-10 diesel UDC at Pacific Pride, indicating a leak in the primary piping. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 91 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This is a repeat violation, Class ll. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by February 21, 2015 . Consent to perform the inspection, take photos and collect samples was given by Ernest Giannecchini the Owner . This facility's designated operator is Karen Arnaiz (ICC expiration date: 5-31-15 ) . The service technician was Zane Nimmo (ICC expiration date: 6-25-15 ), who had the following manufacturer's certifications: VR, 1-29-15, VMI, 1-30-15 .
