A copy of these plans shall be accessible <br />on site at all times. Immediately locate and retain a copy on site. This is a minor violation. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment test on the <br />September 2014, October 2014, November 2014 and December 2014 designated operator monthly inspection report. <br />The secondary containment test dates on the reports were September 23, 2014, the actual test date was September <br />29, 2014. During the monthly inspection, the designated operator shall check that all required testing and maintenance <br />for the UST system have been completed and document the dates they were done. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. This is a minor <br />violation. <br />309 <br />CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the 9-10 at Ernies General Store and 9-10 Pacific Pride UDC sumps failed to stop the <br />flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the <br />chain(s) and verified functionality. This is a Class II violation. <br />314 <br />CCR 2631(a) Primary containment not tight (pre -Jul 2003). <br />There was diesel fuel in the 7-8 and 9-10 diesel UDC at Pacific Pride, indicating a leak in the primary piping. All <br />primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation. <br />323 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 91 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a <br />3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This is a <br />repeat violation, Class ll. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by February 21, 2015 . <br />Consent to perform the inspection, take photos and collect samples was given by Ernest Giannecchini the Owner <br />. This facility's designated operator is Karen Arnaiz (ICC expiration date: 5-31-15 ) .
