Box 357 <br /> Lodi CA 95241-0357 <br /> RE: Three Palms Grocery Tank Removal <br /> The permit application to remove 1 split underground storage tank from the above <br /> referenced facility has been approved with the following conditions. <br /> 1. All analytical results must be taken and tested by a California certified laboratory. <br /> 2. All hazardous wastes must be handled in accordance to California law &/or <br /> regulations. (i.e. treatment and disposal) <br /> 3. All Samples are to be tested for unleaded fuel. (See pages 7-8 of permit) <br /> 4. A copy of the fire department permit must be on site at time of removal. <br /> 5. Schedule removal appointment at least 48 hours in advance. <br /> 6. In addition the following information must be submitted within 30 days of the <br /> removal date: (a) Analytical results, mailed from the lab. <br /> (b)UST Tracking Record Sheet and hazardous waste manifest <br /> (c) Tank Closure Certification Form <br /> (d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or <br /> waste oil receipt. <br /> (e) Submit the backfill excavation certificate as required by <br /> San Joaquin County and the Incorporated City Building <br /> Departments. <br /> 7.
