PUBLIC tEALTH SERN CES
SAN JOAQUIN COUNTY ?,
ENVIRONMENTAL HEALTH DIVISION m. '
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
NOTICE
To: Small Quantity Hazardous Waste Generator Facilities
From: Environmental Health Division
Date: May 25, 1999
Re: Hazardous Waste Generator Program and State Facility Surcharge Fees
On June 25, 1998, the Board of Supervisors adopted a revised Hazardous Generator Fee
Schedule and established a new fee category for Small Hazardous Waste Generator facilities
(generating less than five tons per year). The enclosed billing statement includes the 1999 Small
Hazardous Waste Permit fee of$100.
As the Certified Unified Program Agency (CUPA) for San Joaquin County, the Environmental
Health Division (EHD) is also required to collect a state facility surcharge fee for each facility
regulated under the Hazardous Material Release Response Plans, Hazardous Material
Management Plans, California Accidental Release Prevention Program (Cal-ARP),
Hazardous Waste Generator Program, Tiered Permitting Program or Underground
Storage Tank (UST) Programs. If our records indicate that your facility was not previously
billed for the state facility surcharge of $18.50 for 1998 and/or $10.00 for 1999, these charges
are included on this billing statement.
If used oil is the only hazardous waste generated by your facility, you may be eligible for a 50%
reduction in the Small Hazardous Waste Generator Permit Fee next year. Please complete the
"Used Oil Only" statement on the bottom of this letter and return it with your payment. An
inspection will be performed to verify regulatory compliance and confirm that you are a "Used
Oil Only" small hazardous waste generator.
If you have any questions or require further information, please contact Doug Wilson,
Hazardous Waste Program Supervisor, at 468-3446.
9-C-----------------------------------------------------------------------------------------------------------------
USED OIL ONLY
Facility Name:
Facility Street Address:
City:
Contact Person: Phone:
I certify that the only hazardous waste generated by the above referenced Facility is USED
OIL and that the total amount generated per year is less than 5 tons.
Signed:
0 Division of San Joaquin County Health Care Servi
