• • <br /> SAFE WORK PRACTICES FOR CONTRACTORS WORKING AT RETAIL PETROLEUMICONVENIENCE FACILITIES 29 <br /> 9.4.1.1 General Site Management <br /> • Do not stand underneath a load that is being handled by lifting or digging equipment. <br /> • Stand away from any vehicle being loaded or unloaded,so you will not be struck by spillage or falling materials. <br /> • Workers may remain in the cab of a vehicle being loaded or unloaded if the vehicle has a cab shield that provides adequate <br /> protection. <br /> 9.4.1.2 Surface Crossing of Trenches <br /> Surface crossing of trenches should not be made unless absolutely necessary.When necessary,they are only permitted under the <br /> following conditions: <br /> • Vehicle crossings shall be designed by and installed under the supervision of a Registered Professional Engineer <br /> • Walkways or bridges shall: <br /> — have a minimum clear width of 20 inches, <br /> — be fitted with standard rails, <br /> — extend a minimum of 24 inches past the surface edge of the trench. <br /> 9.4.1.3 Warning System for Mobile Equipment <br /> When mobile equipment is operated adjacent to an excavation,or when such equipment is required to approach the edge of an <br /> excavation,and the operator does not have a clear and direct view of the edge of the excavation,a warning system shall be utilized <br /> such as barricades,hand or mechanical signals,or stop logs.If possible,the grade should be away from the excavation. <br /> 9.4.2 Placement of Equipment, Machinery,and Materials <br /> In no case may machinery,equipment,or materials be placed any closer than two(2)feet from the edge of the excavation.Mate- <br /> rials may not be stockpiled so as to create the potential for material to fall into the excavation.New backfill shall be stockpiled <br /> separately from soils and debris that have been removed forth the excavation. <br /> 9.5 IMPACT OF CHANGES IN WEATHER,SOIL,AND GROUNDWATER CONDITIONS <br /> Whenever there are changes in weather,soil or groundwater conditions,a competent person shall evaluate if the new conditions <br /> require changes to protective systems,or if additional methods are required to safeguard the health and safety of the workers and <br /> protect the environment. <br /> 9.5.1 Standing Water and Water Accumulation <br /> Workers shall have the customer's permission before conducting operations that will require personnel to enter an excavation. <br /> Effort should be made to use techniques that allow the work to be performed without entering an excavation. The following <br /> requirements for water accumulation shall be followed if personnel shall work in the excavation: <br /> • Personnel shall not work in excavations where standing water has accumulated. <br /> • If water removal or de-watering equipment,such as pumps,are used,they shall be installed and monitored by a competent <br /> person. <br /> • Personnel shall exit from excavations during rainstorms. <br /> • Excavations/Trenches shall be carefully inspected by a competent person after each rain and before personnel are permitted <br /> to re-enter. <br /> 9.6 ISOLATION OF THE AREA <br /> Entry to the excavation work area shall be controlled.The excavation site shall be properly isolated to assure that unauthorized <br /> people are not exposed to hazards of the excavation(please see Section 8,Barracading for more information about properly iso- <br /> lating work areas).Specific care shall be taken to assure that the excavation area is not accessible to vehicular or pedestrian traffic <br /> if the excavation is to be left unattended. <br />
