0 • <br /> 34 API RECOMMENDED PRACTICE 1646 <br /> • Rated load capacities. <br /> • Height of lift required. <br /> • Recommended operating speeds. <br /> • Aerial lifts shall be electrically isolated and meet the requirements for insulated aerial devices in OSHA 29 CFR 1926.453. <br /> • A full body harness shall be worn and a lanyard attached to the boom or aerial lift basket when working from an aerial lift. <br /> 10.5.1 Inspections <br /> Frequent and periodic inspections of the lifting equipment shall be conducted in compliance with OSHA regulations.Inspection <br /> documentation shall be readily available for inspection by the customer.This requirement applies to rental equipment as well.The <br /> rental agency shall provide documentation of completed inspections;otherwise,the equipment shall not be used. <br /> 10.6 WORK AREA ISOLATION <br /> See Section 8,Barricading,for a general discussion of work area isolation.When isolating hoisting/lifting work areas special con- <br /> sideration may need to be given for the following: <br /> • Hoisting shall not take place while any person is standing on any part of an object to be lifted,or in a tank excavation. <br /> • Personnel shall never stand under an object suspended by a lifting device or in the travel path of the object. <br /> • Operator shall be aware of the location of adjacent obstacles to avoid trapping personnel between equipment and the obsta- <br /> cles. <br /> • Streets or portions of the public right-of-way may need to be temporarily blocked to ensure public safety.This shall be done <br /> in accordance with all local ordinances and in cooperation with all appropriate governmental agencies. <br /> 10.7 MACHINERY MOVEMENT <br /> Machinery requiring outriggers shall not be traveled while under load.Personnel lift equipment shall not be moved with person- <br /> nel inside of them unless the equipment is specifically designed for that purpose.The traveled area shall be part of the work area <br /> isolation plan and lift plan. <br /> 10.8 TOOL BOX DISCUSSION DIRECTLY PRIOR TO LIFTING ACTIVITY <br /> Before executing the lift plan where there will be more than one worker at a site,the workers shall conduct a toolbox discussion <br /> (tailgate meeting)that includes at least the following: <br /> • work to be performed, <br /> • hazards of that activity, <br /> • conditions that may have changed overnight or since the last time work was performed at the site. <br /> 10.9 RESCUING TOPPLED EQUIPMENT <br /> In the event that a piece of lifting/hoisting equipment topples and needs to be righted,the following shall be considered: <br /> • The event shall be treated as an emergency response and shall be managed by an OSHA trained incident commander. <br /> • A hazard assessment shall be completed. <br /> • There may be a fuel spill associated with the toppling event. <br /> • All applicable emergency response procedures shall be initiated. <br /> • Health and safety of workers and the general public is of primary concern. <br /> • The site owner shall be notified. <br /> • The load shall be secured. <br /> • The entire area surrounding the toppled equipment shall be barricaded. <br /> • A larger crane or lifting device may need to be brought onto the site to right the toppled piece of equipment. <br /> • Local Fire Department shall be notified as deemed necessary. <br /> 10.10 REFERENCES <br /> OSHA <br /> 29 CFR 1926.453 <br />
