No site plan map was submitted into CERS. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a Class II violation. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 4-17-15 and a test report was submitted to EHD on 5-21-15. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection (please refer to violation 106 for the necessary corrections). A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the monitoring system certification on all designated operator monthly inspection reports between April 2015 and March 2016. The previous monitoring system certification testing was conducted on 4-17-2015 and all above reports listed it as 4-27-15. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The 91-octane sump sensor was located on the opposite side from the fuel piping and the test boot(inside the 91 STP sump), and not located to detect a leak at the earliest opportunity. The sump sensor cord was too short to relocate this sensor to the correct sump side. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Correct immediately by having a properly licensed,trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable)and obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. This is a repeat violation, Class II. Page 4 of 6
