A site plan must be submitted identifying the locations where monitoring <br /> will be performed. Immediately log into the California Environmental Reporting System(CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a Class II violation. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on 4-17-15 and <br /> a test report was submitted to EHD on 5-21-15. A copy of the test report must be submitted within 30 days of the <br /> tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely <br /> manner. <br /> This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection (please refer to violation 106 for the <br /> necessary corrections). A copy of these plans shall be accessible on site at all times. Immediately locate and retain <br /> a copy on site. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the correct test dates for the monitoring system certification on all <br /> designated operator monthly inspection reports between April 2015 and March 2016. The previous monitoring system <br /> certification testing was conducted on 4-17-2015 and all above reports listed it as 4-27-15. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91-octane sump sensor was located on the opposite side from the fuel piping and the test boot(inside the 91 STP <br /> sump), and not located to detect a leak at the earliest opportunity. The sump sensor cord was too short to relocate <br /> this sensor to the correct sump side. Monitoring equipment shall be maintained to be able to detect a leak at the <br /> earliest possible opportunity. Correct immediately by having a properly licensed,trained, and certified contractor <br /> replace the failed component with a functional component(LG 113-x listed, if applicable)and obtain a permit within <br /> one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the <br /> 91 UST system may be red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class II. <br /> Page 4 of 6 <br />
