(3) Designated operator employee training log is incomplete. The hiring dates for all employees were missing. Kamal opened the store in Sept 2012. The list shall include dates of training for all facility employees, and the hiring dates for all facility employees hired on/ after July 1, 2005 per CCR 2715(f) (3). Submit to EHD a revised employee training log with hiring dates. 206. (1)The designated operator employee training for Sam was not performed. Sam works primarily on weekends and some week evenings/nights. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. Submit to SJC EHD within 30 days (07/28/13): 1. one copy of the complete "Return to Compliance Certification" form 2. a written corrective action statement for each of the above violations 3. requested or supporting documentation ie. training records Notes: - CERS 10181295 data was entered on site. - Reviewed Financial Responsibility: Certification of Finan Respon. & Letter from CFO dated 06/28/13 - valid until 06/28/14. Must be completed annually. - Designated operator employee Kamal training 04/29/13. Annual training due by 04/14. - Last secondary containment testing done 10/25/10, next due by 10/2013. Provide 48 hr test notification to EHD. ALL EHD STAFF TIME ASSOCIATED MRH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
