<br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed.
<br /> The designated operator failed to document all the alarms from the attached alarm history on the October 11, 2016,
<br /> November 30, 2016, December 5, 2016, February 13, 2017,April 3,2017, May 31, 2017 and the June 2017 print
<br /> out. designated operator(DO) monthly inspection reports and failed to check that they were responded to
<br /> appropriately. The missing alarms include:
<br /> The October 2016 DO report alarm history was not found on site during the inspection.
<br /> On October 28, 2016, a L4 Diesel sump fuel alarm occurred. It was observed that the DO marked"yes"on
<br /> '"sensors located in tank top containment sumps have not alarmed in the past month,"for the November 30, 2016
<br /> DO report.
<br /> The December 5, 2017 DO report is missing an L5 Dispenser 3-4 alarm from the alarm history printout.
<br /> On February 7, 2017, a 1-4 Diesel sump fuel alarm occurred. It was observed that the DO marked"yes"on"sensors
<br /> located in tank top containment sumps have not alarmed in the past month,"for the February 13,2017 DO report.
<br /> No DO was marked for the February 13,2017 DO report.
<br /> On March 22, 2017, a 1-4 Diesel sump fuel alarm occurred. It was observed that the DO marked"yes"on"sensors
<br /> located in tank top containment sumps have not alarmed in the past month,"for the April 3, 2017 DO report.
<br /> On April 16, 2017, a L4 Diesel sump fuel alarm occurred. It was observed that the DO marked"yes"on"sensors
<br /> located in tank top containment sumps have not alarmed in the past month,"for the May 31,2017 DO report. The
<br /> May 31, 2017 DO monthly inspection report alarm history was not verified. The date on the bottom of the tape was
<br /> June 5, 2017. No date was observed at the beginning of the alarm history panel printout.
<br /> On the June 2017 DO report, no"11-3 Regular Middle 5K STP SUMP"was observed on the alarm print out.
<br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check
<br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with
<br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing
<br /> monthly inspections at this facility are including all of the required information on the reports.
<br /> This is a repeat violation, Class 11.
<br /> Overall Inspection Comments:
<br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
<br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting
<br /> paperwork, by November 8, 2017 .
<br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program
<br /> documents into the California Environmental Reporting System (Cli UST Monitoring Site Plan, UST
<br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if
<br /> applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written
<br /> Agreement, if applicable, can be stored at the facility or uploaded into CERS.
