MADISON, STOCKTON, CA <br /> I. CASE SUMMARY <br /> By December 22, 1998, all single-walled underground storage tank systems were required to meet <br /> California Underground Storage Tank Upgrade Regulations or be removed under Public Health <br /> Services, Environmental Health Division(PHS-EHD)permit. The 350 gallon,single-walled <br /> underground storage tank was not upgraded and was found to be storing diesel and in use after <br /> December 22, 1998. <br /> PHS-EHD sent a letter to every underground tank owners in San Joaquin County notifying them <br /> of the imminent deadline in their annual billing statements for the past four(4)years. A survey <br /> was sent in July 1998 to all owners/operators asking how they planned to make their underground <br /> storage tanks comply with the December 22, 1998 deadline. No survey was returned to PHS-EHD <br /> for this facility. <br /> On March 1, 1999, Michael Kith, Sr. R.E.H.S.of PHS-EHD,conducted a routine inspection on <br /> the underground storage tank system at the above location. The tank system contained a single- <br /> walled underground steel tank and single-walled underground steel pipe. At the time of the <br /> inspection,the tank system was in operation and contained diesel that supplied the generator. Ms. <br /> Becky Moreno,Administrator,was advised about the regulation and to either upgrade the tank <br /> system or remove it. An inspection report was left with Ms. Moreno. <br /> On March 12, 1999,Mr. Mike Flores of Stockton Service Station and Equipment submitted an <br /> application for underground storage tank closure permit to PHS-EHD on behalf of owner/operator. <br /> At that time,the tank closure certification form was not completed or signed. The application for <br /> closure permit was approved with conditions on April 5, 1999. On May 21, 1999,Stockton <br /> Service Station and Equipment removed the tank system. <br /> IT.
