MAIN STREET; smc;<Tim <br /> LIFE MEDICAL INDUSTRIES <br />;a The recent soil sampling following the removal of a used oil tank <br /> from the above referenced property was witnessed by a <br /> representative from this department on June 9, 1988. The <br /> presence of petroleum contamination was documented during the <br /> tank closure activities as soil discoloration and petroleum odor <br /> from 5 feet to 11 feet below grade. The field inspection report <br /> noted that the soil sample was collected at approximately 10 feet <br /> below grade. <br /> The owner or responsible party of an underground storage tank <br /> being closed shall demonstrate to the satisfaction of the local <br /> agency that no unauthorized release has occurred (Section 2672 <br /> (d), California Code of Regulations) . <br /> The soil sample results as well as the field observations <br /> indicate that an unauthorized release has occurred at this site. <br /> The extent of possible contamination is unknown, at present, as <br /> is the origin of the soil contamination. Therefore, it will be <br /> necessary for the property owner and/or responsible party(ies) to <br /> conduct a subsurface investigation to define the lateral and <br /> vertical extent of contamination. <br /> Enclosed is our recommended format for "Addressing Soil <br /> Contamination Problems". In addition, pursuant to the San <br /> Joaquin Local Health District fee schedule Resolution No. 85-58, <br /> a $35.00 consultation fee shall be submitted along with the <br /> completed general Environmental Health Application Form for <br /> review of the above referenced environmental <br /> assessment/remediation activity. <br /> Each additional hour over the initial one hour gf.time spent in <br /> review activity will be billed at the rate of $35.00 per hour. <br /> The additional amount due will be billed to the applicant <br /> identified on the general Environmental Health Application Form <br />
