Instead the designated operator checked "Yes"when there was no alarm during that period. <br /> For the same designated operator monthly inspection reports, the designated operator failed to answer#5 <br /> "Sensors located in tank-top containment sumps have not alarmed in the past month."The designated operator <br /> entered the incorrect date for the spill containment testing on the 6/30/16 monthly inspection report; instead of <br /> entering 10/9/15, the designated operator entered 10/8/15. The designated operator failed to check"NA"for#4 - <br /> "Each alarm for the previous month has been responded to appropriately."on the 7/21/16 and 9/26/16 designated <br /> operator monthly inspection report. During the monthly inspection, the designated operator shall check that all <br /> required testing and maintenance for the UST system have been completed and document the dates they were <br /> done. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Approximately one-quarter cup of liquid was observed in under dispenser containment(UDC) 1/2. If water could <br /> enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> The designated operator removed the liquid from UDC 1/2.
