Box 1056 <br /> Linden, CA 95236 <br /> Re: Super Store Industries SR0076033 <br /> 16888 McKinley Avenue <br /> Lathrop, CA 95330 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to replace direct bury Tank-1 diesel spill container with an OPW 1 C- <br /> 2100-DEVR spill container on the underground storage tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Vicki McCartney at (209) 468-9852 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the Tank-1 diesel tank prior <br /> to the EHD inspection and approval. <br /> 4. Since concrete will be broken near the tank, a secondary containment test must be <br /> performed on the tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br />
