<br /> The San Joaquin County Environmental Health Department (EHD) has received the permit
<br /> application on April 10, 2014 to install the following underground storage tank (UST) system at
<br /> the above noted address:
<br /> 40,000-gallon UST, DT1 (ID#TA 0520756) Diesel Grade,
<br /> 40,000-gallon UST, DT2 (ID#TA 0520755) Diesel Grade,
<br /> 10,000-gallon UST, GT1 (ID#TA 0520754) Regular Unleaded Grade,
<br /> Associated piping, dispensers, turbine pumps, appurtenances, and monitoring system
<br /> EHD has completed review of the above permit application. The permit cannot be approved
<br /> until the following have been submitted to EHD for further review:
<br /> 1. Submit Page 3 of the Permit Application with Workers Comp information (including
<br /> expiration date), and Stockton Fire Department Permit# and Building Permit# (or a
<br /> copy of the submitted permit applications).
<br /> 2. Revise and submit Page 8 of the Permit Application with specific sensor models and the
<br /> manufacturer, not just the Monitoring Panel model. Also, an electronic sensor is
<br /> required for the dispenser pan since the facility is a VPH site and there is no drainage
<br /> into secondary containment piping.
<br /> 3. Submit a copy of the Health and Safety Plan complete with diagram and quickest route
<br /> to the nearest hospital.
<br /> 4. Submit blue print with California licensed Professional Engineer stamp, signature, and
<br /> expiration date.
<br /> 5. Submit copies of ICC Technician, ICC Installer's certificates, and manufacturer training
<br /> certifications for each person that installs or tests any of the UST system components.
<br /> 6. Submit the most current manufacturer's installation instruction for Containment Solution
<br /> UST.
<br /> 7. Submit copies of manufacturer's cut sheet for each component of the UST system
<br /> including the fuel systems, fill sumps, piping sumps, vent transition sumps, filter
<br /> transition sump, monitoring system, and any related equipment that require thirty party
<br /> testing.
<br /> 8. Submit third party testing verification of all UST system components per California Code
<br /> of Regulations, Title 23, Section 2631(b).
<br /> 9. Submit Buoyancy calculation signed by a California licensed Professional Engineer.
<br /> 10. Provide drawing of deadmen anchors, turnbuckles, and hold-down straps accurately
<br /> illustrated on the blue print.
<br /> 11. Provide calculation for the flapper drop tube cutoff length that will meet shut off
<br /> requirement for overfill protection per Section 2635(b)(2).
<br /> 12. Neither Sheet FS4.03 nor FS9.01 indicate if the top of the fuel filter transition sumps are
<br /> flush with surface grade, and that the surrounding floor is sloped to the sumps. The
