Miner Ave. , Stockton "Vintage Car Wash" <br /> #1 Permit Revocation: <br /> - FHS-EHD does not revoke permits. <br /> Sites which received permit to operate UGT will remain on <br /> mailing list to receive permits until site inspection <br /> (within 1-3 years per UGT H & S Code) reveals site in <br /> non-compliance. <br /> - PHS/EHD will not issue a permit to a UGT in non- <br /> compliance. <br /> This site received a permit for 1987, 1988, and 1989 <br /> since in 01 status. <br /> Site inspection December 1990 revealed non-compliance and <br /> no permit issued for 1990 . <br /> Site & tanks were, have been in non-compliance since. <br /> #2 On June 18 , 1191, TreaTek, Inc. collect4 subsurface soil <br /> samples by hand augering without workplan approval, of San <br /> Joaquin County boring permit. (TreaTek hired by Pete <br /> Giambanco) <br /> TPH results = 230 mg/kg from 9 feet below ground surface <br /> BTEX samples not analyzed due to laboratory/time <br /> problems. <br /> Total Lead analysis = 160 and 676 mg/kg (PL #1 and FL <br /> samples) <br /> Report not received until June 5, 1992 , without <br /> Appendix A <br /> Directives by PHS/EHD <br /> 1. Eric Trevena's December 4 , 1990 site inspection <br /> documented violations and required corrections by <br /> January 5, 1991. <br /> Conditions not met. <br /> 2. Verbal order by Carol Oz September 25, 1991 directed <br /> Pete Giambanco to submit "Tank Closure Plan" by <br /> October 24 , 1992 . <br /> Conditions not met. <br /> 3 . January 6, 1992, Carol Oz issued verbal/phone <br /> directive with Pete Giambanco to submit "Tank <br /> Closure Plan" by January 24 , 1992 . <br /> Conditions not met. <br /> 4 . "Office Administrative Hearing" held June 10, 1992 . <br /> Conditions not met. <br />
