Main Street <br /> Stockton, CA 95205 <br /> The San ,Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application and subsequent revision dated August 15, 2013 for a permit to break concrete to <br /> expose secondary and primary 89-octane product line in order to locate product leak on the <br /> underground storage tank (UST) system at the above noted address. The permit has been <br /> approved with the following conditions: <br /> 1- Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for review, <br /> comment, and approval prior to commencing work. All new addenda supersede the <br /> previous ones. Submit a service request addendum and all relevant document, <br /> including detailed map/diagram of the repair, and any equipment cutsheet, once the leak <br /> has been located and what needs to be repaired has been determined. <br /> 3. Contact Jeffrey Wong at (209) 209-468-0335 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Secondary piping soap test of exposed fuel lines: One hour pressure test per <br /> manufacturer's guidelines. If there are no manufacturer's guidelines, test shall be <br /> performed using an applicable method specified in an industry code or engineering <br /> standard. <br /> Please be aware that the facility cannot operate the UST system or receive fuel in <br /> the 89-octane tank prior to the EHD inspection and approval. <br /> 4. Since concrete will be broken near the piping (and/or UST), a secondary containment <br /> test must be performed on the near the piping (and/or UST), once concrete is poured in <br /> accordance with the manufacturer's guidelines. <br />
