The Bravo box float and chain in the UDC 7/8 sump (87-octane) failed to stop the flow of product <br /> when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. The service technician readjusted the <br /> float and chain mechanism, retested and it passed. Corrected on-site. <br /> 306. A fuel leak was found at the 89-octane STP sump. Upon arrival approximately three cups of fuel <br /> were observed inside the 89-octane STP sump. The fuel had accumulated at the two lowest points of the <br /> sump as well as inside the sump seam, where the sump joins with the underground storage tank. The <br /> above seam is located approximately 6 inches above the lowest point of the sump and appeared to be <br /> separating (a visible gap of approximately 1/2 -inch was noted) for several feet along the perimeter of the <br /> sump. The technician removed the accumulated liquid from both sides of the sump and identified it as <br /> fuel, a strong odor, and yellowish color was noted. Also the technician was able to insert a small clear <br /> tube (approximately %-inches in diameter) and connected to the hand pump and remove approximately <br /> '/4 to '/12 cup of liquid, which he identified as fuel, from the inside of the sump seam. Furthermore, a small <br /> amount of liquid (approximately 1 to 2 ounces) which could not be readily identified as water of fuel, had <br /> dripped from the open secondary containment pipe. All primary containment must be product tight. <br /> Immediately discontinue use of failed piping, determine the cause of the leak, and have a properly <br /> licensed, trained, and certified contractor replace or repair the failed piping under permit and inspection <br /> of the EHD. If the failed component can't be replaced immediately, there is a possibility that the 89- <br /> octane UST system may be red tagged to prevent fuel inputs. A routine secondary containment testing <br /> is scheduled for June 28, 2013. Submit the Return to Compliance Certification to the EHD along with the <br /> supporting documentation addressing how this violation was resolved, by July 27, 2013. <br /> Minor-Notice to Comply Violations: 113: <br /> 113. A new designated operator was hired on April 23, 2013 and notification was not provided to the <br /> EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. A copy of <br /> the designated operator form was submitted to EHD at the time of inspection.
