SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DEPARTMENT
1868 East Hazelton Avenue, Stockton, California 95205-6232
Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sjgov.org/ehd
CONTINUATION FORM Page: 4 of
OFFICIAL INSPECTION REPORT Date: 06/271/ 3
Facility Address: 3440 E Main Street, Stockton, CA Program: UST
SUMMARY OF VIOLATIONS
CLASS I,CLASS II,or MINOR-Notice to Comply)
305. The Bravo box float and chain in the UDC 7/8 sump (87-octane) failed to stop the flow of product
when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or
stop the flow of product at the dispenser when it detects a leak. The service technician readjusted the
float and chain mechanism, retested and it passed. Corrected on-site.
306. A fuel leak was found at the 89-octane STP sump. Upon arrival approximately three cups of fuel
were observed inside the 89-octane STP sump. The fuel had accumulated at the two lowest points of the
sump as well as inside the sump seam, where the sump joins with the underground storage tank. The
above seam is located approximately 6 inches above the lowest point of the sump and appeared to be
separating (a visible gap of approximately 1/2 -inch was noted) for several feet along the perimeter of the
sump. The technician removed the accumulated liquid from both sides of the sump and identified it as
fuel, a strong odor, and yellowish color was noted. Also the technician was able to insert a small clear
tube (approximately %-inches in diameter) and connected to the hand pump and remove approximately
'/4 to '/12 cup of liquid, which he identified as fuel, from the inside of the sump seam. Furthermore, a small
amount of liquid (approximately 1 to 2 ounces) which could not be readily identified as water of fuel, had
dripped from the open secondary containment pipe. All primary containment must be product tight.
Immediately discontinue use of failed piping, determine the cause of the leak, and have a properly
licensed, trained, and certified contractor replace or repair the failed piping under permit and inspection
of the EHD. If the failed component can't be replaced immediately, there is a possibility that the 89-
octane UST system may be red tagged to prevent fuel inputs. A routine secondary containment testing
is scheduled for June 28, 2013. Submit the Return to Compliance Certification to the EHD along with the
supporting documentation addressing how this violation was resolved, by July 27, 2013.
Minor-Notice to Comply Violations: 113:
113. A new designated operator was hired on April 23, 2013 and notification was not provided to the
EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. A copy of
the designated operator form was submitted to EHD at the time of inspection. Corrected on-site.
Notes: Received copies of the Owner Statement of Designated UST Operator and Certification of
Financial Responsibility.
Provided Return to Compliance Certification and Recording Requirements for Unauthorized Releases.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHUS CURRENT HOURLY RATE.
EHD Inspector:121 , Mau -7(') ReceivedBy: Title:
(iL/ _..
EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM
