The following are Class II (Repeat): 104, 205, and the following is a Minor Violations <br /> - Notice to Comply: 110 <br /> 104 (Repeat). Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Immediately submit a copy of the <br /> financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required <br /> information, and submit for review by the EHD. Complete by 07/30/2014. <br /> 205 (Repeat). The June 2014 designated operator monthly inspection report was not found on site, even <br /> though the inspection was done according to an invoice left by the designated operator. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copy of the June 2014 report to the EHD by 07/30/2014. <br /> 110. Annual leak detector testing was last performed on June 27, 2013 and was due by June 30, 2014. <br /> Testing has not been completed. This test is required once every 12 months. Immediately schedule this <br /> test and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be <br /> charged at our current hourly rate. EHD agrees, at this time, to charge against the money remaining in <br /> the permit SR0069936 first for the follow up inspection. <br /> Notes <br /> Facility operators are required to upload the following UST program documents into the California <br /> Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial <br /> Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST <br /> Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance. <br /> After January 1, 2015, noncompliance with this requirement will be a violation. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector Received By.
