The information on this monitoring program <br /> are conditions of the operating permit. The permit holder must notify San Joaquin County Public Health Services <br /> within 30 days of any changes to the monitoring procedures, unless required to obtain approval before making <br /> changes. Required by Sections 2632(k)and 2641(h)CCR. <br /> Facility Name: Beacon Station#3492 <br /> Facility Address: 470 N. Main St. Manteca California <br /> 1. Describe the frequency of performing the monitoring: <br /> Tank: Continuous monitorin . <br /> Piping: Red Jacket Leak Detectors and sump sensors perform continuous monitoring on product lines. <br /> Dispenser Containment: Bravo Boxes have float switch that tris impact valve. <br /> 2. What methods and equipment, identified by name and model,will be using for performing the monitoring: <br /> Tank: Gilbarco monitors the annular spaces <br /> Piping: Red Jacket Leak Detector and Gilbarco TLS-250 <br /> Dispenser Containment: Bravo Boxes monitored continuous.. . <br /> 3. Describe the location (s)where the monitoring will be performed (facility plot plan should be attached): <br /> Gilbarco T annular space and sump sensors on tanks Gilbarco TLS250 control panel in storage <br /> area. <br /> 4. List the name (s) and title (s) of the people responsible for performing the monitoring and/or maintaining the <br /> equipment: <br /> Kim Crocker Manaqer Dawn Pernsteiner,Area Manaqer•Jim Vanderdoes Zone Manager <br /> 5. Reporting format for monitoring: <br /> Tank: Gilbarco TM4 TLS 250 gives visual and audible warning Manager records daily on inspection log. <br /> Piping: Gilbarco TM4 TLS250 gives visual and audible warnings The Red Jacket leak detector goes into <br /> slow flow. <br /> 6. Describe the preventative maintenance schedule for the monitoring equipment. Note: maintenance must be in <br /> accordance with the manufacturers'maintenance schedule but not less than every 12 months. <br /> Preventative maintenance on all equipment is in accordance with the manufacturers' instructions. All <br /> product lines leak detectors and monitoring a ui ment are tested on an annual basis <br /> 7. Describe the training necessary for the operation of UST System, including piping and monitoring equipment: <br /> If monitoring equipment goes into alarm store personnel are instructed to call Manager and Manager <br /> will call maintenance. <br />
