If the backfill material and/or soil is found to be contaminated it must be disposed of <br />at an approved disposal location. Treatment of soil onsite can only be performed <br />under a hazardous waste treatment permit. <br />2. See pages 7 & 8 of the permit application for required sampling and analytical tests. <br />3. A copy of the fire department permit must be submitted at the time of removal. <br />4. Schedule removal appointment 48 hours in advance. <br />5. Sampling firm shall be an independent third party from the contractor. <br />6. In addition, the following information must be submitted within 30 days of the <br />removal date: <br />(a) Analytical results, mailed from the lab <br />(b) UST disposition tracking record and tank hazardous waste manifest <br />(c) Tank closure certification form <br />(d) Hazardous waste manifest for piping and rinsate <br />7.
