Habte along with a copy of his ICC <br /> certificate. <br /> -Employee training record listing the employees trained by Semu N. Habte on 04/04/07 from this <br /> station. <br /> -DO inspection reports submitted by the previous designated operator, Julie Lee, from May <br /> 2006 to December 2006 along with the alarm history report. Copies will be also mailed to the <br /> site today. <br /> -05/18/07 dated paperwork from Elite IV indicating the diesel fill spill bucket passed the <br /> hydrostatic testing. <br /> The week of June 11, 2007 the Designated Operator and myself will train the store employees <br /> at this facility on best management practices and procedures for retail gasoline outlets. Copies <br /> of the training record will be sent to your office at a later date. <br /> Please note, the Financial Responsibility Verification was submitted and delivered to your office <br /> on 04/23/07 via certified mail # 7003 1680 0000 4385 5882. Attached is a copy of a receipt <br /> delivery returned to ConocoPhillips. I also counseled the Store Manager to keep a copy of the <br /> certification of financial responsibility at all times at the facility. <br /> The new store manager was unable to locate the alarm monitoring records for 3 of the 4 months <br /> indicated in your report: July 2006, September 2006 and October 2006. He has taken over since <br /> February 2007 and unfortunately the previous store manager might have misplaced the reports. <br /> However, I gathered the alarm status history for these 3 months and you can find them <br /> attached. The alarm monitoring record for April 2007 is also attached. <br />
