March Lane, Stockton, CA 95210 <br /> The San Joaquin County Environmental Health Department (EHD)has completed the <br /> review of the application for permit to install the spill bucket at the above noted address. <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Toua Yang at(209)468-3459 to arrange the following inspections <br /> appointments(a 48-hour notice is required for inspection appointment(s): <br /> a. Corrosion protection of the fill riser must be verified prior to backfilling <br /> around the spill bucket. <br /> b. Spill Bucket: Verify minimum 5-gallon capacity and pass a 60-minute <br /> Lake Test or equivalent. Proper drainage or removal must also be <br /> verified. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. Qualified installers must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Installation/Retrofitting certificate issued <br /> by the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or installed. <br />
