The missing alarms include: <br /> March 4, 2015-T: 87 Overfill Alarm <br /> February 20, 2015-T: 87 Overfill Alarm <br /> December 12, 2014 -L1: Dispenser 1-2 Fuel Alarm <br /> All alarms from the alarm history need to be documented in the daily alarm logs. Ensure that all alarms from the alarm <br /> history are documented in the daily alarm log and available upon request by the EHD. <br /> Maintenance and monitoring records for the last three years were not found on site. The 2014 Monitoring System <br /> Certification and 2014 Secondary Containment test results were not found onsite. These records shall be maintained of <br /> site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring <br /> equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations <br /> other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenancE <br /> and monitoring records for the last three years on site and submit copies to the EHD. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the March 2015, February <br /> 2015 and December 2014 designated operator monthly inspection report and failed to check that they were responded i <br /> appropriately. The missing alarms include: March 4, 2015 -T: 87 Overfill Alarm, February 20, 2015-T: 87 Overfill Alarm <br /> and a December 12, 2014 -L1: Dispenser 1-2 Fuel Alarm. During the monthly inspection, the designated operator shall <br /> review the alarm history for the previous month, check that each alarm was documented and responded to appropriately <br /> and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Preet Singh was not performed. The designated operator shall train faci <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 1 <br /> months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
