The following are Minor Violations - Notice to Comply: 104, <br /> 204, 310, 311, 999 (HSC 25404 (e)(4)) <br /> 104. Financial responsibility documents were due on April 9, 2014 and have not been submitted to the <br /> EHD. Current financial responsibility documents are required to be submitted annually. Immediately <br /> submit a copy of the financial responsibility documents to the EHD. Alternatively, log into CERS, upload <br /> the required information, and submit for review by the EHD. Complete by May 23, 20'14. <br /> 204. The designated operator failed to check and note the correct secondary containment test dates on <br /> the April, May, and June 2013 designated operator monthly inspection reports. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> 311. The 87-octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak <br /> detector, retested it, and it passed. <br /> 310. About 1/2 gallon of liquid was observed in the diesel STP sump. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br /> regulations, and manage it accordingly. Liquid was removed for reuse following secondary containment <br /> test. Ensure sumps are maintained free of liquid. <br /> 999. HSC 25404 (e)(4). The California Environmental Reporting System (CERS) information has not <br /> been updated since it was rejected by EHD in April 2013. All business owners are required to submit <br /> current data from their CUPA hazardous materials programs (UST, hazardous waste generator) to the <br /> state electronically pursuant to A132286.
