The UST Facility Operating Permit Application and UST Tank Information is not current in CERS. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log <br /> into CERS, update the required information, and submit for review by the EHD. Complete a separate <br /> tank form for each tank. <br /> 109, 110. Annual monitoring system certification and leak detector testing were last performed on <br /> August 24, 2012 and was completed on November 18, 2013, three months past due. These tests are <br /> required once every 12 months. Take all necessary precautions to ensure testing is performed in a <br /> timely manner. <br /> 201. The November 2011 monitoring system certification test results and the November 2012 spill <br /> container test results were not found on site. These records shall be kept on site for at least 3 years. <br /> Immediately locate and maintain these documents on site. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> July 18, 2013 designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarms include: June 16, 2013, June 15, 2013 - Q1- 87 line <br /> leak detector - gross line fail. During the monthly inspection, the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> lesceaer.
