A new designated operator, Jason Haase, was hired on March 24, 2011 and notification was not <br /> provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 <br /> days. The inspector made a copy of the notification document during the inspection. <br /> 203. Approximately half a pint of diesel fuel was found in each dispenser 9/10 and 11/12 secondary <br /> containments on 11-4-11 (today) and was not recorded. Any release from the primary containment that <br /> does not escape the secondary containment and was cleaned up within eight hours of being detected <br /> shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. A form was provided by the inspector to the operator.
