This is a repeat violation, Class II. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on November 18, 2014 and a test report was submitted on January 2, 2015 , a month and a half late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. A new designated operator was hired, Dale Careaga in December 2014 and notification was not provided to the EHD within 30 days of the change. A designated operator statement was not uploaded into CERS indicating who the designated operator(s)for the facility is. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation, Class Il. 123 CCR 2661 Permit to repair UST system not obtained. The designated operator noted in the July 2015 monthly report that a L2 sensor wire had to be re spliced and a permit was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the L2 sensor that was repliced .
