Immediately log into the California Environmental Reporting System <br /> (CERS)at http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on November <br /> 18, 2014 and a test report was submitted on January 2, 2015 , a month and a half late. A copy of the test report must <br /> be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test <br /> reports are performed in a timely manner. <br /> This is a minor violation. <br /> 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. <br /> A new designated operator was hired, Dale Careaga in December 2014 and notification was not provided to the EHD <br /> within 30 days of the change. A designated operator statement was not uploaded into CERS indicating who the <br /> designated operator(s)for the facility is. Any changes shall be reported to the EHD within 30 days. Immediately log <br /> into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification <br /> identifying all the designated operators for this facility. <br /> This is a repeat violation, Class Il. <br /> 123 CCR 2661 Permit to repair UST system not obtained. <br /> The designated operator noted in the July 2015 monthly report that a L2 sensor wire had to be re spliced and a permit <br /> was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD <br /> prior to beginning work. Immediately obtain a permit from the EHD for the L2 sensor that was repliced .
