Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work.All new addenda supersede the previous ones. <br /> 3. Functionality of sensor must be verified to ensure that monitoring has been restored. <br /> Sensor functionality was verified during the Monitoring System Certification on July <br /> 20,2017, no further testing is required. <br /> 4. If concrete will be broken near the tank,a secondary containment test must be performed on the <br /> tank once concrete is poured in accordance with the manufacturers guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid,current <br /> contractors licenses issued by the Contractors State License Board(may be issued to the <br /> installation company), or a Tank Tester's license issued to the individual by the State Water <br /> Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at <br /> the work site who possesses a valid,current California UST Service Technician certificate issued <br /> by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested, repaired, or serviced. <br /> 6. Individuals installing underground storage tank system components must possess or work under the <br /> direct and personal supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. A valid, current contractors license issued by the Contractors State License Board,and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code Council <br /> (ICC),and <br /> c. A valid,current certificate of training from the manufacturer(s)of the component(s) being installed. <br /> 7. The tank owner or operator shall log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and ensure all UST information is current. <br /> 8. All components must be certified by an independent testing organization and labeled as required by <br /> the organization (UL, ULC,ANSI,API,ASTM,etc.).See California Code of Regulations(CCR)Title <br /> 23, Chapter 16,Table B for a complete list. <br /> 9.
