Under IV. Responsible Persons- remove the <br /> name of Darlene Duff since she is no longer a responsible person. The response plan was revised and uploaded to <br /> the California Environmental Reporting System(CERS)on the day of inspection. Ensure a copy is available for <br /> inspection at any time. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations <br /> where monitoring will be performed. The facility prepared the UST Monitoirng Site Plan and uploaded the site plan <br /> into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/on the day of the <br /> inspection. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 115 CCR 2715(a) Failed to inform the EHD of designated operator identification or change within 30 days. <br /> A designated operator, Brian McPheely, completed a number of designated operator monthly inspection reports but <br /> notification was not provided to the EHD within 30 days of the new designated operator. Any changes shall be <br /> reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> The facility failed to document all of the daily alarms from the Veeder Root panel and failed to check that the alarms <br /> were responded to appropriately. The missing alarms include: Periodic Line Fail for Q4:diesel on 4/8/2017, and <br /> T1:Unleaded Overfill Alarm on 8/5/2016. Ensure that this facility documents all daily alarms from the Veeder Root <br /> panel and responds to each alarm appropriately. <br /> Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed at a lab or in <br /> the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test <br /> results; (6) results of inventory readings and reconciliations. <br /> This is a repeat violation, Class II. <br /> FA0016523 PRO524617 SC001 07/20/2017 <br /> EHD 23-01 Rev.09/22/16 Page 4 of 7 Underground Storage Tank Program OIR <br />
