A "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br /> submitted by the owner or the owners' agent [pursuant to California Code of Regulation(CCR) <br /> 2635(d)] before this upgrade will be considered complete by SJCEHD. <br /> 17. An accurate and drawn-to scale-as-built plan must be submitted to EHD prior to scheduling a final <br /> inspection. <br /> 18. Passing Enhanced Leak Detection (ELD)results must be received prior to the fuel being received. <br /> Note: A separate permit application is required to install ELD. <br /> 19. Contact Michelle Le, REHS (209)468-9847 for inspections. See inspection checklist for the <br /> required inspections. All appointments must be scheduled 48 hours in advance. <br /> 20. This permit expires at the end of the calendar year.
