The EHD will generate and <br />provide a complete report to the facility operator. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />**The inspection could not be completed. The technician was unable to remove the plug under the shear valve in <br />the gasoline dispenser (NR-1) in order to connect the leak detector tester box. The 87 leak detector was not tested <br />This testing is required to be completed at least every 12 months and still needs to be completed. Immediately <br />schedule an inspection for the 87 leak detector test and provide 48 hours notification to the EHD. Please be aware <br />that a follow up inspection will be charged at our current hourly rate. <br />Page 3 of 4
