The DO failed to visually inspect the UST system for leaks, water, or debris during the January, <br /> February, April, July and August 2010 monthly inspection in a timely manner. On 01/28/10, an alarm <br /> history report attached to the January monthly report has an future alarm for 02/03/10. February monthly <br /> report was not written until 03/06/10. April monthly report was not written until 05/04/10. July monthly <br /> report was not written until 08/04/10. August monthly report was not written until 09/15/10. During the <br /> monthly inspection, the DO shall visually inspect all spill containers and UDC sumps, and any tank top <br /> sump that had an alarm for which there is no record of a service visit. Ensure that DOs performing <br /> monthly inspections in a timely manner at this facility are inspecting all of the required areas and noting it <br /> on the on the reports. Submit a statement how facility will ensure monthly reports are performed in a <br /> timely manner. <br /> # 23c. The DO failed to check and note the correct test date for the secondary containment/ SB 989 <br /> testing on the designated operator monthly inspection reports. October 2009 - January 2010 monthly <br /> reports document date as 11/14/08. (11/14/08 was from the annual monitoring system certification, leak <br /> detector and spill bucket testing). February - May 2010 monthly reports document date as11/19/10. June <br /> to September 2010 monthly reports document date as 11/9/10. It is possible that careless handwriting <br /> and copying previous reports was the cause of date error and that test date was not verified from test <br /> results. During the monthly inspection, the DO shall check that all required testing and maintenance for <br /> the UST system have been completed and document the dates they were done. Ensure that DO <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> Submit a statement how to ensure correct secondary containment test will be written and what <br /> secondary containment system test date the DO will document on future monthly reports. <br /> Submit to SJC EHD within 30 days (11/15/10): <br /> 1. one copy of the completed 'Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violation <br /> Notes: SJVAPCD witnessed air quality testing on 10/5/10. It was found that TP201.4 (Methodology 4- <br /> Dynamic Back Pressure) had blockage and failed, low point in vapor line. Island Petroleum technicians <br /> work order documents that repairs and retest needed by 11/4/10 to avoid penalty. To retest TP201.3 <br /> (Press. Decay test), TP201.4, and VL (VR202x5) after repairs. Proposal by Island Petroleum is to re- <br /> slope vapor recovery line from UDC to STP but must break concrete above tanks.
