Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cesar Rubalcaba at (209) 953-6213 to arrange the following inspection appointments <br /> (a 4&hour notice is required for inspection appointments) . Please be aware that inspections <br /> for emergency work must be scheduled within five days of permit approval. <br /> a . Secondary containment testing on under dispenser containment sumps 5/6, 9/10 and 11 /12 <br /> after returning the system to operating condition . <br /> b . Secondary containment testing on the 91 tank annular space after returning the system to <br /> operating condition . <br /> c. Secondary containment testing on the 87 and 91 product piping and spill container <br /> tightness testing on the 91 container after returning the system to operating condition . This <br /> testing is required since concrete will be broken over the 91 tank annular riser and <br /> dispensers 5/6 , 9/10 and 11 /12. <br /> 4. Secondary containment testing for the 91 tank and dispensers 5/6, 9/10 and 11 /12 is required <br /> six months after installation . Provide 48 hours notification to the EHD when this test is <br /> scheduled . <br /> 5 . Within 30 days , the tank owner must log into the California Environmental Reporting System <br /> (CERS) , complete and submit a "UST Certification of Installation/Modification" form for the <br /> primary and secondary containment components installed . <br /> 6. Within 30 days , submit the secondary containment test and spill bucket results on the Title 23 <br /> California Code of Regulations forms to the tank owner and the San Joaquin County <br /> Environmental Health Department. <br /> 7 .
